This patient, fortunately, sustained no major injuries after the car accident, which provided an opportunity for Traditional Chinese Medicine (TCM) treatment.
Outcome:
Yesterday, the second day after the injury, the pain had lessened, but due to going downstairs to use the restroom, swelling occurred above the knee joint. Today, the third day after the injury, the wound has scabbed over, swelling has reduced, and the pain has almost disappeared. The patient can get out of bed but still has difficulty walking.
Efficacy:
I cannot definitively say whether my prescription was effective, for the following reasons:
- The patient is young and did not sustain bone or tendon injuries, so recovery might naturally be faster.
- I lack experience in traumatic injuries. This patient is my nephew, who had an accident due to excessive cycling speed.
Physical Examination:
A laceration was present at the right knee joint, with continuous bleeding. The patient reported severe pain and difficulty moving. There was slight swelling above the knee joint.
Management:
1.Surgical Disinfection:
As I have left clinical surgery, the disinfection process was actually quite non-standard and should be corrected. The correct disinfection method for contaminated wounds is from the periphery to the center, that is, from the relatively aseptic area to the relatively contaminated area.
2. TCM Treatment:
Following Chapter 23 of Jinkui Yaolue (Synopsis of Prescriptions of the Golden Chest), “Formulas for injuries from horse falls and all injuries to tendons and bones”: Wine-processed Rhubarb (Jiu Dahuang) 6g, Peach Kernel (Taoren) 15g, Charred Cattail Pollen (Puhuang Tan) 15g, Charred Human Hair (Xueyutan) 30g, Licorice (Gancao) 15g, Jimo aged wine 50ml.
I removed the difficult-to-obtain herbs mentioned in the original formula, leaving only these.
