[TCM Medical Record]:

A 32-year-old female, slender build, was diagnosed with Diminished Ovarian Reserve (DOR) at the Provincial Hospital of Traditional Chinese Medicine. Ultrasound showed a chocolate cyst. She typically experienced heat in her upper body and cold in her lower body. She had a poor appetite, was prone to abdominal distension, had no chest tightness, low spirits, acceptable sleep, nocturia twice, smooth bowel movements, was prone to sweating, and had early menstruation. Her complexion was yellowish-dark, her tongue was thin with a thin coating, and the tongue body was pale and tender. Menstrual history: menarche at 14 years old, period duration 7 days, cycle 15 days, with prolonged spotting. She had one child previously, approximately 8 years old.

[Initial Prescription after Diagnosis]:

After taking Huangqi Guizhi Wuwu Tang (Astragalus and Cinnamon Twig Five-Ingredient Decoction) with Kidney Four Flavors and a large dose of Cornus officinalis, her appetite improved, spirits were good, hands and feet were warm, she no longer felt upper heat and lower cold, and had no abdominal distension. She continued normal preparation for pregnancy.

[First Consultation]:

October 3, 2024. Pattern differentiation: Taiyin and Shaoyin concurrent diseases. The prescription continued to be Huangqi Guizhi Wuwu Tang combined with Kidney Four Flavors, with modifications: Raw Astragalus (Sheng Huangqi) 20g, Cinnamon Twig (Guizhi) 10g, Raw White Peony Root (Sheng Baishao) 20g, Fresh Ginger (Shengjiang) 3 slices, Jujube (Dazao) 3 pieces (sliced), Cinnamon Bark (Rougui) 5g, Psoralea Fruit (Buguzhi) 20g, Dodder Seeds (Tusizi) 20g, Epimedium (Xianlingpi) 20g, Cornus officinalis (Shanzhuyu) 30g. 5 doses for 10 days.

[Second Consultation]:

October 12, 2024. LMP: October 26, 2024. No abnormalities after taking the medication. This menstrual cycle was 23 days, with no other menstrual reactions. Instructed to continue the previous formula, 5 doses for 10 days.

[Third Consultation]:

November 21, 2024. LMP: November 17, 2024. Considered that Cornus officinalis was not as good as Mume in astringing blood. Prescription used modified Wumei Wan (Mume Pill) without its astringent properties: Mume (Wumei) 20g, Prepared Aconite Root (Pao Fuzi) 10g, Cinnamon Bark (Rougui) 3g, Dried Ginger (Ganjiang) 10g, Sichuan Peppercorn (Huajiao) 10g, Codonopsis (Dangshen) 10g, Angelica Sinensis (Danggui) 10g, Coptis (Huanglian) 5g, Phellodendron Bark (Huangbai) 5g. 5 doses for 10 days.

[Fourth Consultation]:

December 1, 2024. Tongue became thicker and more substantial than before (it seems patients with thin tongues are also difficult to conceive). Prescription: Mume (Wumei) 30g, Prepared Aconite Root (Pao Fuzi) 10g, Cinnamon Bark (Rougui) 3g, Dried Ginger (Ganjiang) 10g, Sichuan Peppercorn (Huajiao) 10g, Codonopsis (Dangshen) 10g, Angelica Sinensis (Danggui) 10g, Coptis (Huanglian) 5g, Phellodendron Bark (Huangbai) 5g. 5 doses for 10 days.

[Fifth Consultation]:

December 10, 2024. LMP: December 10, 2024. This menstrual cycle was 23 days. Prescription: Mume (Wumei) 45g, Prepared Aconite Root (Pao Fuzi) 10g, Cinnamon Bark (Rougui) 3g, Dried Ginger (Ganjiang) 10g, Codonopsis (Dangshen) 10g, Angelica Sinensis (Danggui) 10g. 5 doses for 10 days.

[Sixth Consultation]:

December 22, 2024. Prescription same as before, 5 doses for 10 days.
January 14, 2025. Self-tested positive for pregnancy.

[Seventh Consultation]:

January 18, 2025. Confirmed pregnancy at the hospital. Continued with the previous formula: Mume (Wumei) 45g, Prepared Aconite Root (Pao Fuzi) 10g, Cinnamon Bark (Rougui) 3g, Dried Ginger (Ganjiang) 10g, Codonopsis (Dangshen) 10g, Angelica Sinensis (Danggui) 10g. 5 doses for 10 days.

[Eighth Consultation]:

February 8, 2025. Progesterone recheck: 13. Continued with the previous formula: Mume (Wumei) 45g, Prepared Aconite Root (Pao Fuzi) 10g, Cinnamon Bark (Rougui) 3g, Dried Ginger (Ganjiang) 10g, Codonopsis (Dangshen) 10g, Angelica Sinensis (Danggui) 10g. 5 doses for 10 days.

[Ninth Consultation]:

February 19, 2025. Ultrasound recheck was normal. Prescription: Mume (Wumei) 30g, Prepared Aconite Root (Pao Fuzi) 15g, Cinnamon Bark (Rougui) 3g, Dried Ginger (Ganjiang) 15g, Codonopsis (Dangshen) 20g, Angelica Sinensis (Danggui) 20g, Raw White Atractylodes (Sheng Baizhu) 30g, Taxillus (Sangjisheng) 15g. 5 doses for 10 days.

[Tenth Consultation]:

March 3, 2025. 11 weeks pregnant, ultrasound normal, progesterone: 24. As I was unsure, I asked the patient to take the medicine for another week. Prescription: Mume (Wumei) 30g, Cinnamon Bark (Rougui) 3g, Dried Ginger (Ganjiang) 15g, Codonopsis (Dangshen) 20g, Angelica Sinensis (Danggui) 20g, Raw White Atractylodes (Sheng Baizhu) 30g, Taxillus (Sangjisheng) 15g. 5 doses for 10 days.
